2. Supporting documents required for application for authentication
A. Application form: Completely fill in and sign (our company can fill it in for you, and your signature is required)
B. Two recent photos: One recent 2X2 inch color photo (meets passport photo requirements)
C. Valid passport: Valid for at least 6 months (valid for 6 months from the return trip), with at least three blank visa pages, and please provide a copy of one page of your passport photo
D. Round trip booking or itinerary: Itinerary (including air ticket reservations to and from Europe)
E. Travel Health Insurance: It is necessary to ensure that the date in the Schengen area is within the insurance period. Please note that the minimum insurance is USD50,000.
F. Proof of accommodation: * Hotel/Guesthouse reservation OR * Rental agreement OR * Invitation letter from the host whose home you will be staying at
G. Financial Proof: Bank account statements for 3 consecutive months
H. Visa fees: The consulate charges slightly different fees in different Schengen countries (usually within $100)
I. Others: Proof of identity and address in the United States, such as green card, driver’s license, utility bill, etc.
